Total RewardsPrimary Care is recruiting for an Advanced Practice
Registered Nurse for the role of Nurse Practitioner Faculty for the
Center of Excellence (COE) at the West Haven campus. The incumbent
will be faculty in a post-graduate fellowship program centering on
inter-professional primary care practice. The incumbent will be
responsible for all applicable clinical, administrative,
educational and professional activities as APRN in the primary care
practice. As an APRN Faulty for the COE, the selected applicant
will practice independently and inter-professionally in the primary
care setting; function as a preceptorto nurse practitioner students
and fellows; teach faculty seminars as assigned; and as a
consultant to other healthcare professionals. Duties
include:Functions as a Primary Care Provider for a select panel of
patients in collaboration with the Patient Aligned Care Team (PACT)
members; Independently performs a comprehensive intake on newly
assigned patients to panel including a complete physical exam;
obtains appropriate screening and diagnostic tests based on health
history and physical findings; develops a plan of careincluding
follow-up assessment and evaluation; Promotes the role of the nurse
practitioner as a health professional skilled in collaboration and
facilitation in the PACT model; Models the nurse practitioner role
as a clinical practice resource to other health professionals
orally and in writing; Functions as a preceptor to nurse
practitioner students and fellows; Teaches faculty seminars as
assigned; and acts as a consultant to other healthcare

cases of graduates of foreign schools of professional nursing,
possession of current, full, active and unrestricted registration
will meet the requirement of graduation from an approved school of
professional nursing. OR The completion of coursework equivalent to
a nursing degree in a MSN Bridge Program that qualifies for
professional nursing registration constitutes the completion of an
approved course of study of professional nursing. Students should
submit the certificate of professional nursing to sit for the NCLEX
to the VA along with a copy of the MSN transcript. (Reference VA

graduation from an approved school of professional nursing.Current,
full, active, and unrestricted registration as a graduate
professional nurse in a State, Territory or Commonwealth (i.E.,
Puerto Rico) of the United States, or the District of
Columbia.Preferred Experience:Master's degree in nursing required;
Must have Board Certification from ANCC as a Family Nurse
Practitioner, Adult Nurse Practitioner or Gerontology Nurse
Practitioner; Two years experience in successfully delivering
Primary Care preferred; Excellent assessment skills and priority
setting ability; Good interpersonal and leadership skills;
Experience in precepting nurse practitioner students desirable;
Ability to work collaboratively with the Primary Care team in
meeting performance measures and achieving performance
involvement.Grade Determinations: The following criteria must be
met in determining the grade assignment of candidates, and if
appropriate, the level within a grade:Nurse I Level I An Associate
Degree (ADN) or Diploma in Nursing, with no additional nursing

an ADN or Diploma inNursing and a bachelor's degree in a related
field with no additional nursing practice/experience; OR a
Bachelor's of Science in Nursing (BSN) with no additional nursing
