St. Vincent's Medical Center in Bridgeport has more than 3,200
employees. It includes a 473-bed community teaching hospital, a
76-bed inpatient psychiatric facility in Westport, a large
multispecialty provider group, and special needs services for
adults and children. St. Vincent's is the first hospital in
Fairfield County to be integrated into the Hartford HealthCare
network, and is the system's second-largest hospital.
JOB SUMMARY
Provides temporary practice, pending results of the licensing
examination for a period not to exceed 90 calendar days after the
date of graduation. This temporary practice privilege automatically
ceases upon notification that the graduate nurse has failed the
licensure examination or once the 90 calendar days after the date
of graduation has past and required licensing is still pending.
Provides holistic, quality and cost-effective nursing care for
patients/families and/or others for a patient population within a
unit, service or other primary area of practice with an awareness
of the episode of care. Applies the nursing process within the
context of the organization's Framework for Professional Practice.
Practices under the supervision of a licensed Registered Nurse.
Reports to Unit Manager
JOB RESPONSIBILITIES:
- Assesses and plans patient care under the direct observation of
a licensed Registered Nurse by:
- Assessing and interpreting data accurately
- Identifying and documenting problems and expected outcomes
based on a plan of care for complex, multi-variance population of
patients
- Utilizing scientific knowledge base and appropriate
physiological, psychological, social, cultural and spiritual
concepts
- Organizing care and interventions
- Identifying appropriate resources in order to meet individual
and family health goals.
- Administering prescribed medications using the five rights of
medication administration.
- Coordinates and implements patient-centered care according to a
plan of care by:
- Assisting in establishing and following plans of Health Care
Team and educating patient/family about the plan of care
- Ensuring documentation and communication among caregivers
- Delegating and ensuring satisfactory completion of activities
with Unlicensed Assistance Personnel and peers (when in resource
role)
- Educating patient/family to meet established health goals in
order that all patients/families receive coordinated seamless
care.
- Uses reliable tools and measures. Evaluates and documents
patient/family responses, (including post-discharge/post transfer
follow-up via measurement of nursing-sensitive outcomes).
Identifies complex variances from anticipated results, and
appropriately revises plan of care according to patient/family
needs, monitoring outcomes across shifts for unitP-based stays and
functioning as patient advocate as needed.

- Demonstrating proficiency in basic computer skills such as
operating clinical display and logging on, identification and
navigation of clinical display, utilization of rosters and charts,
troubleshooting and logging off
- Demonstrating a willingness to accommodate all customers at all
times; maintaining an awareness of patient needs and utilizing
initiative to respond to those needs.
- Uses reliable tools and measures. Evaluates and documents
patient/family responses, (including post-discharge/post transfer
follow-up via measurement of nursing-sensitive outcomes).

- Maintaining confidentiality of all patient information, and
Functioning in a collaborative manner with other members of the
team to ensure quality patient care.
- Assumes responsibility for self-improvement, in collaboration
with the leadership team by identifying, communicating and seeking
resources or instruction/validation to meet own learning needs, and
is self-directed in maintaining currency of validations.
- Enrolls in a Nurse Residency Program and engages in activities
that promote a transition into practice.
- Performs other related duties as required Qualifications
Education
- Completion of a formal Nursing program.
- For Associate degree hires, Nursing Baccalaureate Degree (BSN)
must be obtained within six years of your hire date. Licensure,
Certification, Registration
- Application to sit for CT RN license must be completed and
licensing obtained within 90 calendar days of graduation date.
Knowledge, Skills and Ability Requirements
- Demonstrates ability to develop, deliver and evaluate patient
care.
- Basic knowledge of high reliability.
- Strong communication skills, both verbal and written.
- Able to work as part of a collaborative team.
