Job Description:

Assesses, plans, develops and implements with the patient or responsible caregiver a comprehensive home health plan of care for nursing and identifies patient needs for other disciplines and services. Case manages, coordinates and evaluates the plan of care with therapeutic, social service, pastoral care, home health aide services, other organizations and physicians for an assigned caseload. Instructs patient or responsible caregiver in nursing care, disease management, health promotion as directed by the physician's plan of treatment and consistent with Agency regulatory and professional standards.Qualifications:* Baccalaureate or Associate Degree in Nursing or Diploma Graduate* Minimum of one year of recent experience as a Registered Nurse.
