and in need of RNs that want to be part of a collaborative team
environment. We are looking for nurses that have experience and
knowledge of home care which is one of the fastest growing
industries in the country. We are currently seeking: - Full time 32
Hours a week RN Case Managers (flexible schedule) Job Requirements:
- • Comprehensive knowledge and skill in current nursing practice.
- • Knowledge of the concept of team and primary care nursing and
the implementation of both models of care delivery. - • Excellent
knowledge of the OASIS process. - • Knowledge of the community
resources. - • Ability to make meaningful observations, evaluations
and reports. - • Ability to recognize the needs and concerns of
patients, caregivers, and the multidisciplinary team to maintain
therapeutic relationships. - • Ability to provide skilled nursing
care based on scientific principles, the nursing process, basic
behavioral concepts, and intelligent professional judgment. - •
Valid driver's license. • RHMLHHH
