Job Description:
The Health and Welfare Council of Long Island (HWCLI) is seeking
a dedicated Community Health Worker to join our team. This position
plays a critical role in bridging the gap between communities and
healthcare systems to ensure that the health and welfare needs of
Long Island residents are effectively served. The Community Health
Worker will work closely with healthcare professionals and
community members to provide education and support, connecting
individuals with the necessary resources to improve their health
and well-being. Assist individuals and communities to navigate and
access community services, including healthcare, financial
assistance, and social services. Facilitate communication and
client empowerment in interactions with healthcare systems. Conduct
outreach to raise awareness about health issues and resources
available. Gather data on health trends within the community for
analysis and reporting to improve community health strategies. Work
collaboratively with social service providers and health care
providers to tailor health resources to the specific needs of the
communities. Organize and lead community workshops and meetings on
health-related topics. Document client interactions and services
provided, maintaining confidentiality and compliance with all
applicable laws and regulations. Strong interpersonal and
communication skills Ability to work independently and within a
team Knowledge of community resources and health systems Empathy
and sensitivity to diverse populations and cultures Basic data
collection and analysis skills Medical Insurance 401k Certified
Community Health Worker (CCHW) credential preferred. Bilingual
skills (especially Spanish or other languages spoken in the
community) are an advantage
