Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 260 days Shift: 5x5 Days Hours per Shift: Guaranteed Hours not specified Experience: Minimum of 2 years in a home health setting within the last 15 years License: Current RN Licensure in New York State Certifications: Basic Life Support (BLS) required before start date Must-Have: Experience conducting comprehensive assessments of patients Description: Urgent need for professionals in the home health sector. Nurses must have recent experience conducting comprehensive assessments of patients. Candidates will perform initial in-home evaluations of patients. The evaluation includes assessing health status, strengths, care needs, and preferences. These assessments will guide the development of individualized long-term care service plans. Candidates will review relevant medical documentation and health referral forms. Data from evaluations should be entered into electronic forms and transmitted as required. Documentation of any concerns or conflicting information during the evaluation process is essential. Continuity of care is emphasized to reduce fragmentation, duplication, and gaps in treatment plans. Discussions with consumers regarding health care options and necessary supports will be part of the role. Communication with the Call Center for enrollments into managed care health plans will be initiated.
