Job Description Job Description /n /n Assists in
evaluating/reconciling/implementing pharmacy patient care. Performs
activities of a Pharmacy Technician, as needed. /n Job
Responsibility /n /n 1.Collaborates with multiple sources
including, but not limited to, patients, patients’
family/care-giver, outside pharmacies, physician offices,
nursing/assisted-living facilities and/or other hospitals to
obtain/document/organize medication and supplement information
leading to clarification/completion of medication history/current
profile for patients prior to (and/or during) admission. /n
2.Assists in discharge planning including, but not limited to, /n
•Identifying patients unable to afford medication. /n •Identifying
rejected pharmacy claims at patients’ preferred pharmacy. /n
•Initiating prior authorization process. /n 3.Participates in
implementing and evaluating related Quality Assurance, Performance
Improvement and Compliance programs. /n 4.Assists Registered
Pharmacist in the non-professional (non-judgmental) aspects
preparing and dispensing medication. /n •Fills, packages, labels
and stocks unit dose cart from computerized pick list; brings cart
to nursing unit according to predetermined schedule. /n •Makes
other deliveries, as required. /n •Fills bottles or other package
forms with measured amount of medication either manually or
mechanically. /n •Labels containers of drugs according to
department specification, either manually or mechanically. /n
•Assists in clean air room, using sterile technique in filling and
labeling injectable prescriptions, where required. /n •Assists in
preparation and/or labeling of IV additives. /n •Fills medication
orders in inpatient area. /n •Packages sudden, unexpected
medication requests, as required. /n 5.Assists Registered
Pharmacist in maintaining a variety of records and reports. /n
•Maintains IV records with Pharmacist direction. /n •Maintains
records for medication in inpatient area. /n •Maintains records of
preparation of extemporaneous medications. /n •Assists in the
maintenance of statistics and records for all medication orders. /n
•Enters orders into computer terminal for refill purposes. /n
6.Maintains sufficient inventory at workstations in Pharmacy. /n
•Transports required pharmacy material, as necessary. /n •Receives
and maintains records of stock and inventory control. /n •Assists
in material control by maintaining records and inventories. /n
•Maintains placement of sufficient daily inventory of supplies to
all areas of the Pharmacy to assure efficient operating conditions.
/n 7.Performs related duties, as required. /n *ADA Essential
Functions /n Job Qualification /n /n •Associates Degree, required.
/n •Active Certified Pharmacy Technician (CER-PHT) from the
Pharmacy Technician Certification Board (PTCB), required. /n
•Current license as a registered pharmacy technician in NYS,
required when hired on/after 4/25/21. /n •Minimum of three (3)
years retail pharmacy experience required. Retail experience must
be within the last five (5) years. Hospital pharmacy experience
preferred but not required. /n •Strong working-knowledge of
medications including generic/brand names, standard doses, and
classes of drugs used for various conditions, required. /n
•Demonstrates exceptional communication and organizational skills.
/n •Ability to display compassion to patients, families, and
co-workers. /n •Ability to use advanced settings of a variety of
computer programs including standard Microsoft Office applications.
/n •Ability to learn computerized hospital pharmacy system. /n
•Must be able to do physically strenuous work. /n *Additional
