Our facility specializes in post-acute care and rehabilitation, focusing on creating customized healthcare plans to aid in patient recovery and enhance well-being. With a focus on individualized treatment, you will work closely with patients to help them achieve their highest potential for mobility and independence. Evaluate patients’ condition and develop appropriate physical rehabilitation plans Help patients improve their mobility and manage pain through exercises, stretches, and other therapies Assess the effects of therapy and adjust treatments accordingly Educate patients and families about their conditions and what to expect from the recovery process Maintain patient records and document patient progress Collaborate with other medical professionals to plan and modify care Participate in educational or professional development sessions to stay updated with the latest physical therapy practices Strong interpersonal and communication skills Ability to manage multiple patients effectively Proficiency in creating and following through with care plans Hands-on experience with therapeutic exercises and techniques Medical Insurance Dental Insurance Vision Insurance Valid state Physical Therapy license CPR certified Completion of a clinical residency in physical therapy is preferred
