Job Description:
Jersey Shore Post Acute seeks a dedicated Outpatient Physical
Therapist to join our dynamic team. This position is pivotal in
helping patients regain their health and mobility after acute
events. As a therapist, you will work closely with a team of
healthcare professionals to ensure optimal patient outcomes through
tailored physical therapy programs. You will be instrumental in
assisting patients through their recovery phases, helping them
achieve their highest levels of functional independence. This role
requires compassion, dedication, and a robust knowledge of
therapeutic practices and patient care. Evaluate patient conditions
and develop appropriate physical intervention plans. Administer
physical therapy treatments and track patient progress. Document
therapy outcomes and patient records accurately. Collaborate with
healthcare professionals to refine rehabilitation strategies.
Educate patients and families about treatments and home exercises.
Adhere to all professional, local, and federal regulations and
guidelines. Strong interpersonal and communication skills.
Proficient in using physical therapy equipment and techniques.
Ability to plan and execute individual patient care plans. Strong
analytical and problem-solving skills. Medical Insurance Dental
Insurance Vision Insurance Valid Physical Therapy license in the
state of practice. CPR certification preferred.
