This Registered Nurse position is located in Specialty Care Service at the West Haven location. This position is a Full time position at 40 hours per week. [ "Specialty Clinics is recruiting for a full time Registered Nurse to join the clinical team at the West Haven campus. The assignment affords you the opportunity to deliver care to a diverse population in collaboration with a dynamic healthcare team. Specialty Clinics includes medical and surgical subspecialties. Incumbent will support the Home Based Cardiac Rehab, Prevention, and Electrophysiology sections of Cardiology outpatient clinics and will hold general nursing responsibilities as an RN in Specialty Care. Incumbent must be willing to train in all Specialty Clinics as needed and cover medical and surgical subspecialties as assigned The Registered Nurse (RN) in Specialty Clinics uses the nursing process and evidence-based practice in collaboration with the unique Specialist in developing and providing a patient-driven plan of care. The RN in Specialty Clinics is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. The RN provides patient and family health education with a focus on self-management, prevention, and wellness based on the patient's goals. The RN demonstrates leadership by serving as an advocate for patients. The RN works with the Specialty Service/s to maintain access for new referrals and timely follow up. Duties include: Performing initial nursing assessment and prioritizing needs. Developing inter professional plan of care and patient-centered goals based on initial assessment and evaluating patient progress towards goals. Demonstrating knowledge and skills necessary to provide care appropriate to the age of patients served. Evaluating patient response to care. Documenting observations, patient progress toward goals. Assessing basic patient/family/significant other education needs. Reviewing discharge instructions with patients/family. Verbalizing knowledge of and initiating appropriate action in emergencies. Participates in precepting new employees (RN, LPN, NA, HT) and validates unit-specific competency completion.
